What is a Hernia?
The Hernia is a common condition. It is a protrusion through a localized weakness or defect of the abdominal wall or the diaphragm in case of Hiatal Hernia. The Hernia allows the inner lining of the abdomen to push through the weakened area of the abdominal wall to form a small balloon-like sac, which can be felt as a bulge under the skin. This weakness or tear in the abdominal muscles may allow fatty tissue or an organ, such as the intestine, to protrude through the weak area.
What are the symptoms of Hernia?
Hernias are commonly identified by a painful lump under the skin, where the pressure of the tissue pushing through the weakened area can cause a significant amount of pain and discomfort. The hernia is often revealed when lifting heavy objects, coughing, straining during urination or bowel movements, or during prolonged standing or sitting.
What is the Treatment of Hernia ?
Over time, a hernia will increase in size and become more painful; it cannot be treated by medication or external devices and so surgical repair is the only treatment option.
What are the types of Hernias?
There are different types of hernias depending upon their location in the body:
• Inguinal Hernias: Hernias commonly occur in the groin area and are called inguinal hernias. They can be bilateral and are more frequent in men
• Ventral Hernias: They are hernias of the anterior abdominal wall. When they occur at the site of a previous operation they are called Incisional Hernias. When they happen in the belly button area they are called umbilical hernias
• Hiatal Hernias: Occurs when part of the stomach pushes up into the chest through a defect in the diaphragm. It is often associated with heartburn which is the result of abnormal reflux of acid from the stomach into the esophagus
Surgical treatment comes in two forms:
The traditional hernia repair involves making an incision at the site of the hernia which will extend through the skin, fat and muscles until the hernia is reached. The hernia is then repaired by closing the defect with sutures and/or placing a small piece of surgical mesh over the defect. The laparoscopic hernia repair (commonly known as keyhole surgery) is a newer form of surgical treatment. It requires three very small incisions (1cm or smaller) through which are introduced a laparoscope (tiny telescope) and the necessary surgical instruments. The surgeon can then work “inside” the body from behind the abdominal wall. The hernia is by this way repaired and a synthetic patch is placed over the hernia defect and fixed using small surgical staples.
Postoperative Pain
After laparoscopic repair, most patients experience minimal immediate postoperative pain and have little or no need for analgesics after the first postoperative day. Patients are able to leave the hospital within 6-12 hours and return to normal activities within a few days (such as showering, driving, walking up the stairs and lifting objects).
Neuralgia and Paresthesia
It is important to know that during hernia repair, potential injury to small sensitive nerves in the dissected area of the groin may cause neuralgia (pain secondary to nerve injury) or paresthesia (skin numbness). Neuralgia and paresthesia are more common after open mesh repair than after laparoscopic repair.
Recurrence
The overall risk of recurrence with the laparoscopic approach is related to the surgeon’s level of experience. In experienced hands the recurrence after a laparoscopic repair has been reported to be lower or equal than after an open repair
Exploration of the non Symptomatic Groin
During the laparoscopic repair and without the need for any additional incisions the non symptomatic groin area can be explored (from the inside and with high accuracy) for the presence of a hernia. This obviates the need for re-operation and eliminates any further work loss for the patient.
Bilateral and Recurrent Hernias
• When bilateral hernias are present, the open repair requires two incisions (one for each hernia). Bilateral hernias can be repaired laparoscopically without the need for any additional keyhole incisions. • When a recurrent hernia is approached using the open technique the surgeon will be facing a difficult dissection due to severe scaring caused by the previous operation. This is reflected by a longer operative time and higher risk of complications such as injury to the testicular vessels and the vas deference. The laparoscopic approach is performed from the “inside” avoiding all the scaring. This will result in an easier and less risky dissection. • For these reasons the laparoscopic technique has become the method of choice for repairing recurrent or bilateral hernias.
Cosmesis
The cosmetic result of laparoscopic hernia repair is also far superior to that of traditional surgery as only three incisions are performed, one is 1cm long and hidden by the belly button; the other two incisions are only half a centimeter each.
Which Type of Treatment is Best for You?
The laparoscopic inguinal hernia repair is an excellent alternative to the open hernia repair. It should be offered as an option to every patient that is interested in an early return to normal activity. It is the technique of choice for recurrent and bilateral inguinal hernias
